Playsafe 2024/2025 school year

The Playsafe Registration is required each school year. There’s an administration fee of $25 per family due upon registration. Please be sure to complete this registration form before scheduling, and to thoroughly read the parent contract.

If you are filling out a printable version, please correct the school at the top of the page if you are not registering for Dassa McKinney. You can mail it to our office at 320 N. Main St., Slippery Rock, PA 16057, or stop in to drop it off with a payment. Call our office at 724-794-8180 to make a payment over the phone.
